I acknowledge that my/my child’s personal data will be processed, in manual and/or automated form, by the operator of Olimpia Milano Summer Camp , as indicated in the epigraph and by Pallacanestro Olimpia Milano srl with headquarters in Milan, via Borgonuovo 11 , co-owners of the processing, for the purpose of the execution of the contract and that is to process my child’s application for enrollment in Olimpia Milano Summer Camp. The essential and indispensable data for the above purpose are my child’s first name, last name, date of birth, full address, as well as my first name, last name and telephone number . Failure to provide the remaining data on the form will not affect the registration process. The data processors are Pallacanstro Olimpia Milano srl and Mr. Alberto Cecchin domiciled at the manager. I take note that by contacting the aforementioned companies I can know the name of the other data processors. I also take note that the rights of access to the data provided by Article 7 of Legislative Decree 196/2003, including the right to obtain the updating, modification and deletion of my data and those of my child or the right to oppose their processing can be exercised at the data controllers. My identification data and those of my child will be communicated by the data controllers and/or data processors to third parties delegated by the data controllers for the performance of services related to the subject matter of the contract (e.g., freight forwarders, companies performing accounting and billing services, auditing, certification of financial statements, etc.) AND who have signed a confidentiality agreement. I also agree that my/my child’s data may be communicated by the data controllers and/or data processors to the persons in charge of processing operations aimed at collecting, recording, organizing, storing, consulting and processing the data.
